NDIS Record Keeping Requirements Explained

Keeping proper documentation isn't just about good admin — it's a core part of NDIS compliance.

✅ What Documentation Is Required?

The NDIS Quality and Safeguards Commission expects providers to keep the following records:

Support logs and progress notes

Evidence of services delivered, outcomes, and participant responses.

Incident reports

Any accident, injury, or behavioural concern — no matter how minor — needs to be documented.

Participant files

Including consent forms, risk assessments, goals, service agreements, and support plans.

Staff compliance

Copies of qualifications, police checks, training records, and First Aid certificates.

Financial records

Invoices, timesheets, payment claims (especially if NDIA-managed).

🕓 How Long Do You Need to Keep Records?

According to the NDIS Commission:

📅 Retention Periods

Participant records

Must be kept for 7 years after the last interaction.

Incident reports

Must be retained for at least 7 years.

Financial records

Should follow the ATO's recommendation: 5 years minimum.

Even if a participant leaves your service, you're still responsible for preserving their information securely.

🔐 How Should Records Be Stored?

You don't need to use paper folders anymore (thankfully), but you do need:

Secure digital storage with role-based access
Audit trails (e.g., when was the note written or edited)
Version control for updated care plans or documents
Automatic backups

This is where quality NDIS software helps — letting you meet requirements without drowning in admin.

🧾 Why Good Record-Keeping Matters

Compliance

Failing an audit due to missing or outdated notes can lead to sanctions or deregistration.

Participant Safety

Up-to-date records support continuity of care and reduce risk.

Efficiency

Clean, structured documentation saves time and reduces duplication.
